Travel


Traveling as a Student Organization

Traveling as a registered student organization can be a rewarding experience; professionally, academically, and socially. To ensure a successful trip, preplanning of all excursions is not only necessary but is required.

Authorization to Travel

All student organizations planning to travel must file a Student Pre-Travel Trip Form, which must be signed by their faculty/staff advisor, a Contact Sheet for every traveler, a Medical Liability Waiver for every traveler, and a Student Acknowledgement of Rules Form, which must be signed by each student going on the trip. The Student Pre-Travel Trip form is available on the ULM Online Forms website located at http://www.ulm.edu/forms/sa/StudentPreTravelForm.doc and the Student Acknowledgement of Rules Form is located at http://www.ulm.edu/forms/sa/StudentAcknowledgRules.pdf.

All trips should be supervised. If traveling in a University owned vehicle, the vehicle is covered under university insurance. If students are traveling in their personally owned automobiles, then the Certificate of Privately Owned Automobiles must also be completed. (See below). This form shall remain on file at the department level. The original may be requested if a claim is filed.

Personal Vehicles

If a driver wishes to use a personal vehicle, a Student Personal Vehicle Wavier must be completed prior to travel dates from the ULM Online Forms Website at http://www.ulm.edu/form/sa/StudentPersonalVehicleWaiver.pdf. All other travel forms and paperwork can be found at http://www.ulm.edu/forms/#studentaffairs. All drivers must include a copy of their valid driver’s license and current car insurance with the forms, as well as a certificate of completion for the Driver Safety Course, which can be found here: http://www.ulm.edu/safety/program_driver.html
